Certified Nursing Assistant (CNA)

Position Summary:

  • The Certified Nursing Assistant (C.N.A.) is responsible for providing quality routine daily nursing care to residents according to the residents’ care plan and under the direction/delegation of the LPN or RN. The C.N.A. will effectively interact with residents, family and other health team members while maintaining all standards of professional nursing.

Essential Functions:

  • Provides individualized attention, which encourages each resident’s ability to maintain or attain the highest practical physical, mental and psychosocial well-being.

  • Attends to the individual needs of residents, which may include assistance with grooming, bathing, oral hygiene, feeding, incontinent care, toileting, colostomy care, prosthetic appliances, transferring, ambulation, range of motion, communicating or other needs in keeping with the individuals’ care requirements.

  • Observes residents for changes in condition or behavior and promptly reports these changes to appropriate licensed nursing personnel.

  • Performs various tasks assigned by the charge nurse using established procedures, policies, guidelines and standards. Including checking vital signs, weighing residents, applying creams/ointments and collecting specimens.

  • Assists with orienting residents and their families to the nursing home on admission and to the unit when transfers occur.

  • Completes all required resident charting every shift.

  • Works collaboratively with other members of the health care team in coordination of the resident’s care.

  • Performs other duties as assigned.

Work Environment: While performing the duties of this job, the employee is in skilled nursing facility atmosphere that has controlled heat and cold. The noise level is moderate to quiet. The C.N.A. may be exposed to residents with contagious illness and must follow established protocols regarding infectious disease control.

Physical Demands:

  • While performing the duties of this job, the employee is regularly required to talk and hear.

  • This position requires a variety of activity levels including frequent standing, walking, bending, kneeling, stooping, crouching, occasional sitting, rarely crawling and climbing. The employee may be required to frequently lift items up to 50 lbs.

  • Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.

Education / Experience:_

  • High school diploma required or equivalent/GED

  • CNA certification required

  • Demonstrated experience within a skilled nursing facility and ability to provide proof of such experience.

Certified Geriatric Assistant

Responsibilities:

  • Assist nursing staff with completion of resident activities of daily living, bathing, dressing, oral care, and other basic needs

  • Perform vital signs on residents at beginning of each shift and as directed

  • Answer calls for assistance and observe changes in resident’s behavior, emotional state, or physical condition

  • Complete required documentation in a timely manner

  • Assist in transporting residents as needed

  • Provide companionship to residents

Registered Nurse (RN)

You Will (Responsibilities)

  • Demonstrate knowledge of home health and competencies in discipline-specific patient care skills required by the care center to provide patient care.

  • Perform comprehensive patient assessments, as assigned, and collaborate with a multidisciplinary care team to develop or implement an individualized plan of care as per physician orders that ensure quality, proper discharge planning, and achieve desired outcomes and goals.

  • Maintain continuity of patient care by communicating information timely and effectively with all team members providing care to the patient.

  • Provide patient episode management per protocol.

  • Completes documentation timely, accurately, and at the point of care, according to industry standards, conventions, and guidelines, including OASIS assessments, SOC/Admissions, Recertifications, ABN’s, subsequent visits, physician orders, care coordination, etc.

  • Educate the patient, caregivers, and family on the aftercare or ongoing care of the patient

  • Perform vital signs and patient assessment during visits

You Have (Qualifications)

  • Current and unencumbered license to practice as a Registered Nurse in the state of Maryland or eligible compact state licensure

  • At least one year (+ 1) of Certified Home Health experience as a RN

  • Current CPR certification.

  • Comfortability using technical tools and software/EMR for visit documentation

  • A reliable method of transportation, current driver’s license, and car insurance coverage

  • Previous experience completing home health OASIS visits

  • Proof of Immunization/Exemption: Hep B, MMR, Varicella, Flu, and COVID-19

  • TB Test or Chest X-Ray

  • A willingness to complete additional training, as assigned
